Good resources will be shared with a larger proportion of people, who will have increased access to a wider range of validated resources, and more accurate, relevant and up-to-date community service listings.  This will lead to increased community connection/engagement, reduced isolation, improved knowledge, skills and confidence to self-manage a health condition, and improved knowledge of healthier lifestyle choices.  As a result, the outcomes will be:

  • Greater awareness of available preventative and self-management resources

  • Improved health and wellbeing outcomes for the people of Leeds

  • Improved self-management for those living with long term conditions

 Referrers:

Due to citizens and practitioners being able to better utilise reliable, up-to-date and trusted online information, which is easy to interrogate, the outcomes will be:

  •  Increased awareness of the service provider landscape.

  • Improved confidence when signposting a person to other services knowing that information has been checked for accuracy.

  • Greater awareness of available preventative and self-management resources.

  • Improved confidence to share resources with those that they support.

  • Improved service user relationships.

  • Reduced research time for referrer such as social prescribers and other advocates.  A reliable source of all relevant information will free up that time to extend their support to the growing list of vulnerable and at risk groups.

 

Organisations and service providers:

Due to having a clear process for organisations to feed information into, a single/universal view of what users are searching for and what searches do not result in a timely return, the outcomes will be:

  • Increased third sector control of information.

  • Increased opportunity for the third sector to share their services and resources with a wider audience.

  • Increased efficiency, therefore more time to dedicate to value-add operational tasks such as charitable activities/serving the citizens of Leeds.
